How to claim
Step 1
Fully complete the Claim Form, completing all of the mandatory fields.
Step 2
Once you click submit, the completed Claim Form will be emailed to you at your nominated email address.
Step 3
Send the following:
- One (1) completed claim form printed out from your email Inbox
- One (1) proof of purchase (clear photocopied invoice)
- One (1) proof of product (original barcode cut neatly from the product box)
- One (1) business size stamped self-addressed envelope

We must receive your claim within 30 days of the date on your proof of purchase.
*Failure to include a fully completed Claim Form and a proof of purchase and a proof of product and a stamped self-addressed envelope will result in your claim being deemed invalid.
*Claims received outside of the 30 day claim period will be deemed invalid.
*Invalid claims will not be issued a Cashback Cheque and will not be issued with an additional opportunity to claim.
Step 4
Within 28 days of receipt of a valid claim, qualifying claims will be posted a $200 Cashback cheque.
The cheque will be drawn in the name of the person on the Claim Form and cheques must be deposited within 6 months of the issue date.
